Join the Practice
If you have chosen to register with our practice then please consider arranging the appropriate registration documents as soon as is convenient. Please do not wait until you are unwell as this will save a great deal of stress and potential anxiety from having to commence the paperwork at a time when you may be feeling ill.
Once registered, your previous medical records are retrieved from your previous doctor; this is so that we can have a full and meaningful medical record.
Live within the Practice's boundary area?
The area in the middle of the interactive map (highlighted in green) below is the main Practice boundary area.
Patients who are already registered in the main Practice area but have moved to the outer boundary area (highlighted in yellow) can continue to be registered as a patient at Blackthorn Health Centre.
However, any newly registered patients living in the outer limits area will not be able to register at the Practice. This also includes patients who have been registered at the Practice before and have since left the Practice. Unfortunately, if your address is within the outer boundary or beyond, we are unable to re-register you at Blackthorn Health Centre.
This is due to a number of reasons including the availability of home visits, community services and out-of-hours care.
If your address falls outside of our practice area, you will need to register at a GP Practice closer to your address. You can search for local GP Practices via the NHS Find a GP search feature.
Please note, that the practice does NOT participate in the NHS Out of Area scheme. As a result, we are unable to accept registrations from patients outside of the Practice's catchment area.
Register at Blackthorn Health Centre
Before you can make an appointment, process a prescription, etc., you will need to register at the Practice. You can register at Blackthorn Health Centre using the link below.
* This registration process, as well as the NHS England registration process, is managed by Healthtech and is the Practice's preferred new patient registration process. You can also register by attending the Practice and requesting the paper-based registration form.
Once you have completed the above online registration form, you will need to complete the Practice's health questionnaire. This is an essential part of the registration process and is required for all of our new patients, as it helps us to promptly meet your medical requirements. If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
We are required to provide all of our patients with a named GP.
Your named GP is the GP who is responsible for handling your paperwork (prescriptions, letters, etc.), and has overall responsibility for the care and support you receive.
This does not prevent you from seeing any GP at the practice.
If you would like to know who your named GP is, please call on 023 8045 3110 or ask at reception.
If you have a preference for your named GP, please ask at reception and we will make reasonable efforts to accommodate you.
